Healthcare and community partners

Partner with Novin.

A responsive private home-care option for professionals helping Toronto families plan safe, dignified support at home.

Built for clear handoffs

Families deserve a next step they can understand.

Novin provides private personal support and home-care services. We can receive an introduction, explain our process, assess non-clinical support needs, and communicate service decisions clearly—with the individual's consent.

Who we work alongside

Practical collaboration. Clear boundaries.

  1. 01

    Hospitals and discharge teams

Discuss non-clinical home support that may complement an appropriate discharge plan, including personal routines, meal support, companionship, and family respite.

Service begins only after client or authorized representative consent, Novin assessment, and confirmation of fit and availability. Novin does not make discharge decisions or replace required clinical services.

  2. 02

    Physicians and primary care teams

Offer patients and families a private home-support option when day-to-day assistance may help them follow established routines and remain connected at home.

Clinical assessment, diagnosis, treatment, and medication decisions remain with the appropriate regulated professionals. Information is shared only with valid consent and a defined purpose.

  3. 03

    Retirement homes

Explore supplementary one-to-one personal support or companionship for residents when the request fits the residence’s policies and the individual’s service plan.

Any arrangement requires clear role boundaries, residence approval where applicable, and written service expectations. Capacity and caregiver availability are not guaranteed.

  4. 04

    Social workers and community professionals

Give families a clear point of contact to discuss private personal support, respite, and companion care alongside public and community resources.

An introduction is not an acceptance of service. Novin will speak with the client or authorized representative, assess the request, and explain next steps directly.

A considered process

From introduction to a confirmed care plan.

  1. 01

    Introduction

Share Novin’s contact information, or—with valid consent—invite us to contact the client or authorized representative.

  2. 02

    Conversation

We listen to the person’s goals, routines, timing, and non-clinical support needs.

  3. 03

    Assessment

We determine service fit, clarify boundaries, and review caregiver and scheduling availability.

  4. 04

    Confirmation

Care begins only after the parties agree to a written service plan and practical arrangements.

Start a conversation

Discuss a thoughtful pathway for the people you support.

An initial conversation is exploratory and creates no referral, service, exclusivity, or contractual commitment. Services remain subject to consent, assessment, fit, availability, and written agreement.
